The article presents information on a high frequency oscillator (HFO) as the best lung protective ventilatory approach for ARDS. The first patent for HFO was issued to Jack Emerson, the designer of the Emerson ventilators, in 1959. Subsequently, interest in the use of high frequency ventilation has gone through a number of cycles of increased and decreased enthusiasm for the approach. Over the last 10 years, the impact of the mechanical ventilator on lung injury has been studied extensively and the approach used to provide conventional mechanical ventilation has been shown to have an impact on outcome in two randomized controlled trials
