ABSTRACT
The article offers information on the Medicare Quality Improvement Organization in Rhode Island which was awarded a Medicare contract. It highlights the improvement of the safety of patient care through the Safe Transitions Project by translating effective patient and provider interventions into sustainable systems change. The article also mentions the Quality Partners of Rhode Island that collaborated with physicians, health plans, and community leaders. A chart is presented which list the best practices in the medical setting.
